Certified birth certificate, valid U.S. Government issued pictured ID card and valid Social Security CARD



How To Apply:

State law requires that a certified copy of each party's birth certificate or birth card be presented with correct spelling of names. If either party was born out-of-country, a translated notarized birth certificate along with a valid state ID card or driver's license must be presented to our office.

After 30 days
